Green Form

This online Green Form consists of two parts.

Part 1 Club Section.
The secretary completes this part and by using the "submit form" button, sends this part to the Demonstrator/Judge/ Teacher/Speaker. There will be an invitation on the email send to the Demonstrator/Judge/Teacher/Speaker to open up their part of the form.

Part 2 Demonstrator/J/T/S Section
The Demonstrator/J/T/S will receive the email containing the link to open their part of the Green Form.
The Demonstrator/J/T/S completes Part 2 and by using the "submit form" button sends this part to the secretary.

Both the secretary and the Demonstrator/Judge/Teacher/Speaker will receive details of both Part 1 and Part 2 of the form in an email.

Guidelines for printing and posting Green Form.
1. Download and print both forms.
2. Fill in the Club Section of the Form and post this to the Demonstrator including the Demonstrator Section of the Green Form.
3. Demonstrator, check details on Club section and fill in your section accordingly. Take a copy of both forms and post originals back to the Club.
