Green Form

This online Green Form consists of two parts.

Part 1 Club Section.
The secretary completes this part and by using the "submit form" button, sends this part to the Demonstrator/Judge/ Teacher/Speaker. There will be an invitation on the email send to the Demonstrator/Judge/Teacher/Speaker to open up their part of the form.

Part 2 Demonstrator/J/T/S Section
The Demonstrator/J/T/S will receive the email containing the link to open their part of the Green Form.
The Demonstrator/J/T/S completes Part 2 and by using the "submit form" button sends this part to the secretary.

Both the secretary and the Demonstrator/Judge/Teacher/Speaker will receive copies of both Part 1 and Part 2 of the form.

When a new Club Committee is selected, you will need to notify AOIFA so that we know who to talk to

What information do you need to have to fill in the Form?

Your club's AOIFA membership number.
New Chairperson, Secretary and Treasurer's Name, Email address and phone no.

You will receive a confirmation and copy of the form after you submit the form.

What information do you need to have to fill in the Form?

Treasurer's Name, Email address and phone no.
Number of members in your club.

What is this for?
All Demonstrators/J/T/S
have to be individual members of AOIFA.
Club members can also be an individual member and receive relevant AOIFA information.

What information do you need to have to fill in the Form?

Just your information, ie. your club name, your name, email address and phone no.

You will receive a confirmation and copy of the form after you submit the form.
